Pathway Development Service - Yorkshire and Humberside

We don’t offer direct therapeutic work with service users, but we can support teams with reviews, training, and team thinking spaces.

Training and team thinking spaces can be done as standalone or following a case review. If you are not sure what would be most helpful, don’t worry! Complete one of our referral forms and we can think about this together. Visit the referral information page for more information.

Reviews

  • Case review: An independent, holistic review of a service user’s needs and pathway, which may include identifying relational and systemic barriers.
  • Housing and resettlement reviews: An independent, holistic review with a focus on community pathways including discharge from hospital or sustaining a community placement.
  • Joint reviews: A combination of the above two reviews exploring both inpatient and community options.

For all reviews we will produce a report which will include recommendations to support future pathway planning.

Training

The training we offer has been co-produced with lived and occupational experience with a focus on trauma-informed care, relational practice, boundaries, formulation, and working with complexity.

Team thinking space

Facilitated spaces for teams to think together about their work, dynamics, and challenges. This could be related to a specific service user or may include themes that arise from the work the team is undertaking for example dilemmas, disagreements, or stuck points.

Self-help guide for friends, family and supporters

We have also created a booklet developed by carers, for carers. The booklet is a self-help guide for friends, family and supporters of those who may have difficulties consistent with a ‘personality disorder’ diagnostic label.
