Asket Croft and Asket House
Asket Croft and Asket House are specialist community-based rehabilitation units that provide a stepped care approach to inpatient rehabilitation. This offers flexibility to meet the individual needs of the service users and enables people to define, build and live a meaningful, independent and satisfying life.
- Asket Croft: 20-bedded mixed-gender community inpatient unit that provides a supportive setting for service users in the early stages of their recovery who have a higher level of need.
- Asket House: 16-bedded mixed-gender community inpatient unit that provides a more independent setting for service users with a lower level of need and who are further on in their recovery but still require 24-hour support provided by the multidisciplinary team.
We offer a multi professional, multi-agency approach to helping people with rehabilitation needs. Our teams consist of doctors, psychologists, nurses, occupational therapists, recovery workers, peer support workers, healthy living advisors, dieticians, pathway inclusion workers, harm reduction workers, and administration staff.
- We provide community inpatient settings that are safe and supportive environments that support independence.
- We promote sustained recovery for living independently in the community, not simply being ready to move back to the community.
- We promote the practice of assessed and safe therapeutic risk taking to enable greater independence.
- We aim to reduce the risks associated with service user presentation and keep their admission as short as possible.
- We work from a Multi-Disciplinary Team (MDT) perspective, considering service users’ needs from a holistic perspective.
- Our review meetings (MDT meetings) are conversation led, which means that we consider everyone in the meeting to have an equal say in understanding service users’ distress. We aim to openly talk about different perspectives or differences in opinion together with the service user always present.
- We offer a range of interest-based activities (for example art group, gardening) and skill development groups (for example cooking groups, communication skills) throughout the week to support social integration and promote meaningful activity, routine and structure.
- We work closely with service users, their relatives, and carers to fully understand a person’s needs/risks.
- Understanding a person’s physical health needs and how this may impact their mental health is integral to our ongoing assessments.
- Psychology is an integral part of our model; all service users have a formulation to better understand a person’s presentation and help us formulate care plans.
- We provide holistic care packages delivered by the right people/services through joint partnerships with third sector organisations.
- We work with service users through the pathway and transition into the community.

Level 2 Rehabilitation – Ward 5, Newsam
Newsam Centre Ward 5 is a 17‑bed, male-only high intensity rehabilitation ward for adults aged 18–65. It is the only NHS high intensity rehabilitation facility in West Yorkshire, serving the Leeds area of the Integrated Care Board (ICB).
Our aims and objectives:
- Symptom minimisation through holistic recovery-focused treatment (mental, physical, social, vocational and educational needs).
- We offer a range of interest-based activities (for example art group, gardening) and skill development groups (for example cooking groups, communication skills) throughout the week to support social integration and promote meaningful activity, routine and structure.
- We work closely with service users, their relatives, and carers (where appropriate) to fully understand a person’s needs/risks.
- Understanding a person’s physical health needs and how this may impact their mental health is integral to our ongoing assessments.
- Psychology is an integral part of our model; all service users have a formulation to better understand a person’s presentation and help us formulate care plans.
- Independent living by enabling service users to gain practical life skills and promote autonomy.
- Close community collaboration to ensure safe, meaningful transitions into supported or independent living.

Assertive Outreach Team
The Leeds Assertive Outreach Team (AOT) is a specialist community mental health team for people over the age of 18, living in Leeds, with a primary diagnosis of psychosis. Psychosis is when people lose some contact with reality.
Our team supports people with complex and long-term mental health problems. They may have been admitted to hospital before; they may have health issues from drugs and alcohol use. They also may have received care from other mental health services, but the treatment didn’t suit their needs. This is when we can help.
Our approach is different. Service users and their carers will work with us to help us understand them as an individual. We think creatively about what support we offer. We make sure the care and support is flexible to adapt to each person’s needs. We will support recovery and independence so our service users can live a fulfilling life and avoid admissions to hospital or reduce the length of a stay, if its required.
Your care will be provided by a range of professionals on our team including:
- doctors,
- psychologists,
- social workers,
- occupational therapists,
- nurses,
- support workers,
- healthy living advisors,
- harm reduction workers,
- accommodation gateway workers, and
- administrative colleagues.
We work together with our partners and other services, such as Forward Leeds, Community Links, and Touchstone, to provide the best possible care.
Once someone has been referred to the Leeds Assertive Outreach Team they will have a meeting with a member of the team to get to know them, understand what their physical and mental health needs are, and what recovery means to them.
The multi-disciplinary team will then meet to review the care and support the service user will receive. We take all our colleagues’ expert perspectives into account and keep them at the centre of the discussion to ensure the care is personal.
Support may include:
- Psychotherapy
- Crisis management support
- Weekly social activities like football, art group, allotment group, or music group to provide a routine and structure
- Working with other services, partnerships, or charities to provide treatment or activities
- Support for carers or family members
- Support to help you reduced drug and alcohol use
The team provide support in the community and home environment. Our aim is to work with service users to avoid hospital admission. If a stay in hospital is required, we will continue to work with them throughout their stay.
During recovery, we will regularly review the support plan with the individual and their carers and adapt the plan whenever we need to.
Once someone is ready to move on from our service, we’ll ensure that the necessary support networks are in place to help people live good quality fulfilling lives.

Community Rehabilitation Team
The community rehabilitation team is a specialist community mental health team that supports adults and older adults in their homes, communities and supported living environments to their optimum level of functioning and independence.
The team provides rehabilitation interventions in provides person-centred, strengths-based, and recovery-focused approach that supports individuals across their recovery journey, from inpatient care to community living. It promotes hope, confidence, and purpose through collaborative, compassionate, and evidence-based care. Personalised interventions empower autonomy, skill development, independence, and reconnection with communities which enhance wellbeing, quality of life, and sustained recovery while reducing reliance on inpatient services
The team is a multi agency and multi disciplinary to provide you with the support you need, the different roles you might work with include
