Complex Psychosis Pathway
The complex psychosis pathway includes several services working together to provide a whole system approach. Enabling people with experiences of psychosis and complexity that impact on their day to day functioning and quality of life. To access the care they need when they need it through a city-wide pathway. Individuals may enter the pathway at any of the services and may move in and out of different parts of the pathway as their needs change throughout their recovery journey.
What We Offer
Our pathway brings together specialist services in Leeds, including:
- Assertive Outreach
- Community Rehabilitation
- Complex Psychosis Practitioner Community Hub offer
- Level 1 Inpatient Rehabilitation (Asket House and Asket Croft)
- Level 2 Inpatient Rehabilitation (Ward 5 Newsam, relocating to Parkside Lodge)
- Out-of-Area Case Management and Mental Health Supported Accommodation Placement Review (in collaboration with the Local authority)
Non clinical services that remain available for the whole population of Leeds while also working closely with the pathway are:
- Recovery College and Arts and Minds
- Volunteers Service
These services work together as a whole system pathway to ensure you get the right care, in the right place, at the right time without having to tell your story multiple times.
