Complex Psychosis Pathway

Who can refer

Referrals to all services in the Complex Psychosis Pathway will be made through a
a referral form on the Trust’s Electronic Patient Record system, CareDirector, from 19th January 2026.

We do not accept self-referrals.

Eligibility criteria

  • People are over the age of 18 years with a primary diagnosis of psychotic illness referred to as complex psychosis.
  • For people under the care of secondary mental health services and registered with a Leeds GP.

Complex psychosis is defined by NICE Guidance as a primary diagnosis of a psychotic illness (this includes schizophrenia, bipolar affective disorder, psychotic depression, delusional disorders and schizoaffective disorder) with severe and treatment-resistant symptoms of psychosis and functional impairment.’ (NICE, 2020)

Referral process

  1. Referrer completes and submits the detailed referral form via CareDirector.
  2. Referrals are triaged by clinicians on a daily basis and allocated for assessment by practitioners in the Rehabilitation and Assertive Outreach Teams.
  3. Assessments are completed by clinicians from the Assertive Outreach Team and Rehabilitation Teams

Required documentation with referral

Information required from referring services to inform triage and assessment to be available on electronic patient record system or provided by referrer:

  • Service user (and carer, where applicable) awareness and level of agreement to referral, where possible
  • Reason for referral including mental state, rehabilitation needs and the persons view on the  referral
  • Recent risk assessment and safety plan
  • Engagement including current level of engagement and what has been tried to engage the person
  • Any reasonable adjustments or lone working considerations for assessment
  • Recent Mental health act tribunal or hearing reports or summary of contact with services and admissions, if available
  • Occupational Therapy Assessments, if available
  • Psychological Formulation, if available
  • Safeguarding reports, if appliable
