EMERGE Leeds: Complex Emotional Needs Service

have developed ways of coping and surviving that can cause them harm and distress, and we provide a range of different services to meet the needs of people at different stages of their personal development.

These are:

  • Care coordination, split into two pathways, for young people and over 25s
  • Group work programmes
  • Case consultation

The information below is for you if you’re a professional working with someone with complex emotional needs. It is here to help you make sure that your service user gets the most from our services.

Making a referral to our services and group programmes
Care coordination for young adults (18-25)

To access this service, a young person must meet the criteria for intervention from a Community Mental Health Team (CMHT) in Leeds. Referrals should be made via the relevant CMHT using the Leeds and York Partnership NHS Foundation Trust Single Point of Access (SPA).

Care coordination for over 25s

The Journey Programme

People can refer themselves to the Journey Programme, or referrals can come from a health, housing or third sector professional.

Case consultation

EMERGE Leeds will offer case consultation to professionals working in Leeds, including NHS and voluntary sector colleagues.

Case consultation involves an offer of up to four sessions with colleagues to support their work with individuals with complex emotional and interpersonal needs or those with a diagnosis of personality disorder.

Colleagues within Leeds and York Partnership NHS Foundation Trust can make a referral for case consultation via CareDirector. If you are external to the Trust, please complete and return our case consultation referral form.
