The Rehabilitation and Recovery (R&R) Service is a specialist community-based rehabilitation service comprised of two mental health hospital wards (Asket Croft and Asket House) and one community team (Recovery Centre). The service provides a stepped care approach that offers flexibility to meet the individual needs of the service users and enables people to define, build and live a meaningful, independent and satisfying life.

  • Asket Croft: 20-bedded mixed-gender community inpatient unit that provides a supportive setting for service users in the early stages of their recovery who have a higher level of need.
  • Asket House: 16-bedded mixed community inpatient unit that provides a more independent setting for service users with a lower level of need and who are further on in their recovery but still require 24-hour support provided by the multidisciplinary team.
  • Recovery Centre Community Service: The team works in partnership with Leeds MIND, Touchstone and Community Links and is the hub of the R&R service that provides care coordination through the R&R pathway and supports the transition and reintegration of service users into the community.

We offer a multi professional, multi agency approach to helping people with rehabilitation needs. Our teams consist of doctors, psychologists, nurses, occupational therapists, recovery workers, peer support workers, healthy living advisors, dieticians, pathway inclusion workers, harm reduction workers, and administration staff.

  • We provide community inpatient settings that are safe and supportive environments that support independence.
  • We promote sustained recovery for living independently in the community, not simply being ready to move back to the community.
  • We work with service users over the age of 18.
  • We promote the practice of assessed and safe therapeutic risk taking to enable greater independence.
  • We aim to reduce the risks associated with service user presentation and keep their admission as short as possible.
  • We work from a Multi Disciplinary Team (MDT) perspective, considering service users’ needs from a holistic perspective. 
  • Our review meetings (MDT meetings) are conversation led, which means that we consider everyone in the meeting to have an equal say in understanding service users’ distress. We aim to openly talk about different perspectives or differences in opinion together with the service user always present.
  • We offer a range of interest-based activities (e.g. art group, gardening) and skill development groups (e.g. cooking groups, communication skills) throughout the week to support social integration and promote meaningful activity, routine and structure.
  • We work closely with service users, their relatives, and carers to fully understand a person’s needs/risks.
  • Understanding a person’s physical health needs and how this may impact their mental health is integral to our ongoing assessments.
  • Psychology is an integral part of our model; all service users have a formulation to better understand a person’s presentation and help us formulate care plans.
  • We provide holistic care packages delivered by the right people/services through joint partnerships with third sector organisations.
  • We work with service users through the pathway and transition into the community.
