About the service

Our Dementia Mental Health Liaison Practitioners support people who are registered with a Leeds GP and who receive care from their local neighbourhood team.

We can help people aged 65 and over who are experiencing a decline in their mental health, changes in how they function day‑to‑day, memory problems, or other symptoms linked to dementia.

We can also consider referrals for people under 65 who are experiencing cognitive or functional decline, impairment, or frailty. This includes younger people who have dementia or signs of frailty.

What to expect from our service

Our three practitioners work across 13 Integrated Neighbourhood Teams and are linked to four older people’s community localities. The service operates Monday to Friday, from 9am to 5pm, excluding bank holidays.

We aim to:

  • Provide community‑based mental health support for people aged 65 and over. We will also consider referrals for people under 65.
  • Offer access to treatments that are based on the best available evidence.
  • Support people with their dementia and/or mental health needs in a whole‑person, holistic way.
  • Work closely with professionals across Leeds to make sure people receive joined‑up, community‑focused care.

What we offer

Our Dementia Mental Health Liaison Practitioners work closely with service users, carers, families, neighbourhood teams, and other professionals across Leeds to provide:

  • Assessments
  • Interventions
  • Advice, education, and signposting to other services

Their main role is to understand each person’s needs, agree goals with them and their carers, and provide appropriate interventions based on National Institute for Health and Care Excellence (NICE) guidance. They also review progress, make referrals to other services where needed, and keep in regular contact with the teams involved in the person’s care.

Practitioners also develop their practice through joint working, attending relevant meetings, and providing specialist input at neighbourhood case management meetings. They help strengthen working relationships between local services and secondary mental health teams by offering training and sharing expertise.

They support people to access community and third‑sector organisations that offer information, advice, group sessions, and peer support and also provide education, advice, and coping strategies to service users, working closely with:

  • Neighbourhood Teams
  • Memory Assessment Service
  • Community Mental Health Team
  • Intensive Home Treatment Team
  • Memory Support Workers

Referral information

Our Dementia Mental Health Liaison Practitioners work closely with Neighbourhood Teams and their partners to help join up physical and mental health care.

You can make a referral directly to the practitioner who works with your local Neighbourhood Team. Referrals and advice can also be requested internally by staff within the Trust.
