How we can help
We support people who may be experiencing:

Our services

Younger People with Dementia Service
Assessment, diagnosis and support for adults generally up to age 65 who have dementia or dementia‑related symptoms.
Memory Assessment Service (MAS)
Assessment, diagnosis and early treatment for dementia and mild cognitive impairment; support to understand your diagnosis and plan next steps.
Older People’s Community Mental Health Teams
Home‑based support for people aged 65 and over (and some adults under 65) who have complex or long‑term mental health needs.
Older People’s Care Home Teams
Mental health and dementia support for people living in care homes across Leeds, including help with complex needs and behaviour that others may find challenging.
Specialist Interventions and Therapies
Psychology, occupational therapy, medication review, physical health support, and practical guidance to help you stay safe, active and connected.
Referral Information for Professionals
Clear guidance on referral routes, criteria and contact details for GPs, social workers, community teams and other health and care professionals.

We aim to offer person‑centred, compassionate and timely support. Our teams include mental health nurses, psychologists, occupational therapists, psychiatrists, support workers and dementia specialists.
