Adult Attention Deficit Hyperactivity Disorder (ADHD) Service
We are currently not accepting any new referrals, except in the following cases:
- People who have been turned away by other providers due to high risk.
- People currently receiving acute mental health services (including crisis services) within Leeds and York Partnership NHS Foundation Trust (LYPFT).
- Young people transitioning from the Child and Young People’s Mental Health Service who need adult secondary care.
For people below the age of 18, please refer service users to community paediatricians, which is part of the Leeds Child and Young People’s Mental Health Service at Leeds Community Healthcare NHS Trust.
GP’s making a referral under the exceptions listed above
Where patients meet these criteria or where there is an extreme risk to their patient where ADHD is a significant factor, email the ADHD Service to discuss howe we can provide clinical expertise.
- Complete the referrer form for professionals.
- Ask the person you’re referring to complete the service user self-assessment form.
- Include both documents in the same email to the ADHD Service.
Please note, for those completing the service user form if you have difficulties with reading and writing and are struggling to complete the form, then please discuss with the professional referring you. Thank you for your co-operation and understanding.
