• Help planning safe discharge and follow‑up care

What we offer

• Supportive case discussion and clinical advice
• Joint assessments for complex cases (when appropriate)
• Flexible Microsoft Teams discussions with wider teams
• Not all discussions will lead to admission, advice and guidance may be the most appropriate outcome

Discharge and care coordination

Each admitted patient is supported by an RMS Coordinator who completes a holistic assessment, identifies gaps and support needs, and works with local and regional partners.

Supporting families

We offer partner assessments to provide a safe space for partners to explore support needs and receive signposting to appropriate services.
