Community Mental Health Pharmacy Team
Referrals can only be accepted by a primary care prescriber, such as a GP, advanced clinical practitioner, or GP practice pharmacist.
We are working to a transformed model of mental health care. This means we work closely with the wider members of the Community Mental Health Team and the Primary Care Mental Health Team, as well as various third-sector services. We accept re-directed referrals from these teams and may re-direct referrals to or work jointly with these teams as appropriate with the aims of streamlining referrals to mental health services and improving patient care.
Referral criteria
Referral criteria to the Leeds Community Mental Health Pharmacy Team:
- Leeds residents receiving care under the NHS.
- Ages 18 through 65.
- Person has consented to receive input from the Community Mental Health Pharmacy Team.
Exclusion criteria
We cannot accept a referral:
- if input from other Leeds and York Partnership NHS Foundation Trust specialist services is more appropriate.
- for routine reviews of mental health medications.
- for service users who are acutely unwell in their mental health and / or actively suicidal.
- if a service users is under the care of Forward Leeds or in active addiction.
- if the diagnosis is unclear.
For service users who are acutely unwell, under the care of Forward Leeds or in active addiction, or if the diagnosis is unclear will be re-directed to appropriate teams.

Complete the Single Point of Access form. We aim to respond within 10 working days.
