Community Learning Disability Teams (CLDT)
We have three Community Learning Disability Teams (CLDTs) delivering services across Leeds.
The teams are made up of professionally qualified staff including; psychiatrists, psychologists, physiotherapists, dietitians, occupational therapists, speech and language therapists and learning disability nurses.
On 25 September 2017, we launched a new team structure and service criteria following a review. You can read more about this in our news section.
Assessment and Referral Team
This team manages referrals, makes decisions on whether they meet our criteria and which professions would need to be involved.
Community Learning Disability Teams: East North East and West and South team
These teams incorporate the full provision of interventions for community learning disabilities including therapies at the Ventures service and specialist knowledge for those with complex multiple impairments.
Health Facilitation Team
The Health Facilitation Team work strategically with services and professionals across the city, supporting them to make reasonable adjustments and to understand the health needs of the learning disability population. They are a small team which consists of clinical and non-clinical staff who share easy-read resources via the Get Checked Out Leeds website.

How to refer
Referrals for community learning disability services go through our Single Point of Access (SPA) Team.
