Aspire operates an open referral system, meaning anyone can refer to our service.

Eligibility criteria

To be accepted, the person being referred must:

• Be aged 14 to 65.
• Consent to the referral.
• Not have received previous treatment for psychosis, unless this was within the last 12 months.
• Be registered with a Leeds GP or are living in Leeds.

Please note: If the person being referred does not consent to a referral, but you believe a referral would be in their best interests, please contact our Duty Team to discuss and agree a plan.

Referral form

Colleagues at Leeds and York Partnership NHS Foundation Trust (LYPFT) can make a referral through CareDirector. If you are an external professional or you are referring yourself, or a friend or family member, please use the referral form below.

If you are referring yourself, or a friend or family member, and would prefer to do this by phone, please contact the Aspire Duty Team on 0113 200 9170. A duty worker is available by phone Monday – Friday 9am – 5pm.
