Your voice on safety planning At Leeds and York Partnership Foundation Trust (LYPFT) we are reviewing our current way of safety planning. To help us improve how we approach conversations around risk and safety, we are inviting you to share your feedback. Whether you’ve been directly involved in safety planning, supported someone through it, or simply have ideas on how we can do better — your voice matters. All responses will be treated confidentially and used to inform service improvements across the Trust. Feedback is open until the 19 December 2025, and can be shared online or through services across the Trust.

What is a Safety Plan?

A safety plan is a personalised plan developed with an individual (and sometimes their carer or support network) to help keep them safe during times of distress or crisis. They can look different but often cover questions like:

  • Things that have caused me to struggle in the past
  • Things that make me feel unsafe
  • Early warning signs when I start to struggle
  • Things I, others, and services can do to help

 

Why Your Voice Matters

We want to understand how our current way of safety planning is working for the people who use our services and the family/ friends/ carers who may support them. Your feedback will help us understand what is working well and what isn’t.

 

Our Commitment to you

All answers will be read and feedback taken on board. The overall results will feed into discussions about what changes need to be made to how we currently do safety planning. Some changes, like updates to the safety plan form, may happen sooner. Other changes, like how we work together to make safety planning more meaningful and supportive, will take more time. We’re collecting feedback until 19 December 2025, and we’ll share an update on the key themes from your responses through our services and in another news article.

Alternatively, if you are under the care of one of our services please ask them for the Your Voice on Safety Planning pack, which offers some alternative ways to give feedback.
