The CONNECT Adult Eating Disorder Service are moving to Microsoft Forms for referrals

We’re pleased to announce that CONNECT Adult Eating Disorder Service is leading the way at the Trust by becoming the first to use Microsoft Forms for their referral process.

This improvement will make referring patients quicker and easier for everyone involved. By using Microsoft Forms, certain key sections must be completed before the referral can be submitted. This helps to prevent incomplete referrals, meaning fewer delays and less time spent chasing missing information.

The new online form is now live and ready to use. Visit the Eating Disorders Service’s information for healthcare professionals to access the form.

One of the biggest advantages is that the Team can update the form instantly when needed, without having to access every GP system individually. This ensures the process stays up to date and as efficient as possible.

Please can we kindly request that any old downloaded versions of the CONNECT referral form are discarded and switch to the new Microsoft Forms version.

We hope this change makes referring to CONNECT smoother and faster for both referrers and patients. Thank you for your cooperation and support as we roll out this improvement.
