At Leeds and York Partnership NHS Foundation Trust, we’re proud to celebrate the incredible work of our teams and individuals who go above and beyond to deliver compassionate, high-quality care. Here are our latest winners for Team of the Month, Individual of the Month.

Team of the Month – CRISS East

We’re delighted to recognise CRISS East as our Team of the Month for November 2025. This team has shown exceptional strength, unity, and commitment to delivering high-quality, person-centred care during challenging times.

The nomination highlighted how the team consistently puts service users at the heart of everything they do. Despite the pressures of a busy service, they have adapted their ways of working to meet growing demands, tailoring care to individual needs while maintaining flexibility and efficiency. Their forward-thinking approach has improved the delivery of care and created opportunities to enhance quality.

New starters have repeatedly shared how welcomed and supported they feel, reflecting the team’s inclusive and positive culture. This environment of kindness and respect has helped boost morale and build resilience across the workforce. The team’s ability to collaborate and streamline processes has led to smoother operations and stronger coordination, ensuring care remains accessible and efficient.

As the judges commented:

“Excellent examples of a team coming together through a challenging time.”

At the heart of CRISS East’s success is compassion. They not only prioritise service users but also care for one another, creating a culture of mutual support and trust. Their honesty, accountability, and transparency have strengthened confidence among colleagues and service users alike.

Individual of the Month – Charlotte Kelly

We’re proud to celebrate Charlotte Kelly as our Individual of the Month for January 2026. Charlotte has demonstrated exceptional professionalism and dedication in her role as a Band 5 nurse, earning recognition from colleagues and judges alike.

Charlotte was nominated for her outstanding performance during a particularly challenging shift. She was responsible for coordinating care for several young people, both on-site and off-site, when patient plans unexpectedly changed. Despite the complexity and pressure of the situation, Charlotte adapted quickly, reorganising the shift and ensuring that care remained safe and responsive.

Her calm and structured approach made a real difference. Charlotte broke down tasks into clear, manageable steps and communicated effectively with both staff and young people. This helped reduce anxiety within the team and ensured everyone understood what needed to happen next. Her ability to lead with empathy and clarity stood out as an example of best practice.

As her nominator said:

“Seeing her in action made me reflect how fortunate I am to have such a passionate individual in my staff group.”

The judges praised Charlotte for her integrity, creativity, and adaptability. They described her as a person-centred practitioner who demonstrates inclusive communication and sets a positive example for others. Her actions reflect the values we strive for across our services: safe, responsive care delivered with compassion.
