The draft West Yorkshire and Harrogate Sustainability and Transformation Plan (STP) has been published today (Thursday 10 November).

The draft West Yorkshire and Harrogate Sustainability and Transformation Plan (STP) has been published today (Thursday 10 November).

It sets out the vision, ambitions and priorities for the future of health and care in the region, and is in response to guidance published by NHS England in December 2015, which asked every health and care system to come together to create their own plan for the delivery of NHS England’s Five Year Forward View.

It looks at how the challenges presented in the Five Year Forward View will be met by 2020/21, and how to close the following three gaps:

Health and wellbeing:

Pockets of deprivation and affluence across the area means where you live can determine your life chances. The STP aims to address this health and wellbeing gap with a focus on supporting people to live longer, healthier lives, and ensuring a good and equitable service for all – no matter where you live.

Care and quality:

The draft proposals also stress the importance of improving people’s health, through better coordination of services, whilst improving the quality of care received.

Finance efficiency:

An ageing population and people living longer with complex health and social care needs means demand is increasing faster than resources. The STP offers an initial view of how local and regional services can be improved, what this means for the health of people locally, and how collaboration will be necessary to balance the books.

Sustainability and Transformation Plans will be delivered by local health and care ‘geographical footprints’. There are 44 footprints across England, of which West Yorkshire and Harrogate is one.

The vision:

The vision for West Yorkshire and Harrogate is for everyone to have the best possible outcomes for their health and wellbeing.

The plan:

The West Yorkshire and Harrogate STP is built from six local area place-based plans; Bradford District and Craven, Calderdale, Harrogate and Rural District, Kirklees, Leeds and Wakefield, which all focus on the health and social care needs of local people.

There are nine area-wide priorities:

  • Prevention
  • Primary and community services
  • Mental health
  • Stroke
  • Cancer
  • Urgent and emergency care
  • Specialised commissioning
  • Hospitals working together
  • Standardisation of commissioning policies

“We are an active and supportive partner of the draft West Yorkshire and Harrogate Sustainability and Transformation Plan and the local plan for Leeds and we welcome their publication. I am pleased to see that mental health features prominently in the plans and there are some good aims to aspire to.

“It’s important to say that the plans as they stand now are only the beginning of the journey. The ambitions and goals stated in the plans are things that we are already working towards in many areas – for example through our work on improving access to urgent care and crisis services as part of the West Yorkshire vanguard project.

“At a local level, we are already taking steps to integrate our services with primary care, social care and community partners to support those with long-term conditions to cope with anxiety or depression, reduce A&E attendances and hospital admissions. This work has been brought into focus through our recent rapid improvement events where partners from across Leeds have come together with the single aim of improving services for patients.

“At regional level and beyond, it is clear we have to do more to work as one health and social care team and we are already in discussions about how we can do this effectively.”

Involving, engaging and consulting with you

STP partners will continue to work with staff, stakeholders and the public to build the plan, ensuring the involvement of everyone in future conversations around the draft proposals.
