Introducing Patient Hub: Your Secure Digital Portal for Easier Care Management

We’re introducing Patient Hub, our secure digital portal designed to make it easier for you to manage your care with Leeds and York Partnership NHS Foundation Trust.

Patient Hub gives you 24/7 access to important information and tools to support your care. It’s part of our commitment to improving the experience for people who use our services, making communication quicker, clearer, and more convenient.

What can you do now?

  • Receive and complete questionnaires about your health
  • Access information securely from your smartphone, tablet, or computer

What’s next?

This is just the start. Over the coming months, we’ll be adding more features, including the ability to view your upcoming appointments in one place and access more of your care information online. The Trust is on the road to connecting Patient Hub to the NHS App too. We’ll keep you updated as new functionality becomes available.

We’re also maintaining more traditional forms of contact for people who would prefer not to use Patient Hub. You are in control of whether we contact you via the Trust’s digital Patient Portal – contact your care team to update your communication preferences at any time.
