Patient Hub: Your Digital Patient Portal
What is Patient Hub?
Patient Hub is a digital tool designed to help you to stay informed and involved in your care. It’s simple to use on your smartphone or computer, and has a familiar look and feel if you’ve used the NHS App before.
Through Patient Hub, you’ll be able to:
- Complete forms and questionnaires online – before or after appointments, at a time that suits you.
In future, you’ll also be able to:
- Access helpful information – including tips on looking after yourself while waiting for care.
- View appointments and link to the NHS App – making it easier to keep track of your care.
You’ll receive a secure link by text or email when we invite you to use Patient Hub. Using it is always your choice, and all our usual ways of contacting you will remain in place. You can set your contact preferences via your care team – please contact them directly to update these if you prefer one method or want to opt out.
Why are we introducing Patient Hub?
We want to give you more options for how you engage with your care. Some people prefer digital tools for communication, others prefer traditional methods like care in person, over the phone, or by letter. Patient Hub is a new addition, not a replacement for these options.
By using Patient Hub, you can:
- Save time by completing questionnaires online.
- Share this information in your own time and space.
- Help us understand what matters to you .
Our Approach and Plans for the Future
We’re introducing Patient Hub in stages so we can learn and improve as we go.
Phase 1 (2024–2025)
Our starting focus is on questionnaires; short, recognised healthcare surveys called patient reported outcome measures, which provide a picture of your health, wellbeing and day-to-day life, and how these are changing over time.
Phase 2 (2025–2026)
- We’ll be introducing appointment information and resources to support your wellbeing whilst you wait for treatment or assessment by one of our care teams.
- Work to link Patient Hub to the NHS App.

Explore more features to make your care experience easier and more connected.
Your Choice Matters
Not everyone will want to use a digital portal, and that’s okay. We’ll continue to offer all our usual ways of sharing information and staying in touch. Patient Hub is about giving you more choice, not less.
