Complex (Locked) Rehabilitation – Newsam Centre Ward 5

The Complex Rehabilitation service specialises in rehabilitation and recovery work with people with serious mental health issues that significantly impact their health and social functioning. 

The most common referral sources will be via acute inpatient wards, forensic wards, and out of area locked and forensic services. Referrals for Inpatients are made via Care Director, alongside a detailed referral form, providing all relevant clinical information sent to complexrehabreferrals.lypft@nhs.net inbox.

The Complex Rehabilitation Case Manager manages referrals into the service. They will triage, identify the suitability of the referral and discuss it in the monthly Out of Area panel meeting. 

The ward aims to have the assessment outcome and feedback to the referrer and service user within 28 days.

When considering making a referral to the service, the service user must meet the following criteria:

The individual:

  • has a primary diagnosis of mental illness, which includes psychotic features
  • is currently under the care of secondary mental health services
  • is over the age of 18

Their social functioning is so restricted they have trouble leading a meaningful life 

OR

Their independent living skills are so restricted that they prevent them from functioning in a community setting without a high level of maintenance. 

OR

Despite an intense period of active treatment, they continue to experience mental health symptoms daily, which cause them significant distress.
