If you’re already receiving care from Leeds and York Partnership NHS Foundation Trust, then you will have a contact for your Care Co-ordinator or another health professional in the team looking after you.These contact details might be written in your care plan.
Changes to services during Coronavirus Response
To ensure we can continue to provide our most critical services during the Coronavirus pandemic, we will be making some changes to the way we provide some of our community and specialist services. A summary of all these changes can be found on this web page. If you are a service user, please be assured we will be in contact with you directly to discuss any changes that affect you.
Keeping in touch with loved ones
While visiting is suspended we have provided help to service users with zoom web calls to loved ones and we have also introduced a new email delivery service.
We would like to know the impact of Coronavirus pandemic on you. We would like to hear from people who use our services, the carers of people who use our services, or those who have used them in the past, as this will influence the services in the future.
Your participation in the survey is completely voluntary and all of your responses will be kept confidential and your care will not be affected in anyway.
