Rehab and Recovery Services
Referrals to the Rehab and Recovery Inpatient Service (Asket House and Asket Croft) can be made for service users under the care of secondary mental health services.
The service specialises in working with people who have ongoing complex mental health needs which significantly impact their health and social functioning. Due to the nature of the service providing rehabilitation in an open community setting, service users who are referred to the service must be able to access the community safely without an escort.
Referrals are made to the Recovery Centre on Care Director (Electronic Patient System). The following provides a ‘how to guide’ for referring to the Rehabilitation and Recovery Service.
Rehabilitation and Recovery referral.
Once a referral has been received, service users are allocated a care coordinator who will complete an assessment with the service user, carer and care team. This will usually take place within five working days.
