Assertive Outreach Team

Once someone has been referred to the Leeds Assertive Outreach Team they will have a meeting with a member of the team to get to know them, understand what their physical and mental health needs are, and what recovery means to them.

The multi-disciplinary team will then meet to review the care and support the service user will receive. We take all our colleagues’ expert perspectives into account and keep them at the centre of the discussion to ensure the care is personal.

Support may include:

  • Psychotherapy
  • Crisis management support
  • Weekly social activities like football, art group, allotment group, or music group to provide a routine and structure
  • Working with other services, partnerships, or charities to provide treatment or activities
  • Support for carers or family members
  • Support to help you reduced drug and alcohol use

The team provide support in the community and home environment. Our aim is to work with service users to avoid hospital admission. If a stay in hospital is required, we will continue to work with them throughout their stay.

During recovery, we will regularly review the support plan with the individual and their carers and adapt the plan whenever we need to.

Once someone is ready to move on from our service, we’ll ensure that the necessary support networks are in place to help people live good quality fulfilling lives.
