Assertive Outreach Team
The Leeds Assertive Outreach Team (AOT) is a specialist community mental health team for people over the age of 18, living in Leeds, with a primary diagnosis of psychosis. Psychosis is when people lose some contact with reality.
Our team supports people with complex and long-term mental health problems. They may have been admitted to hospital before, they may have health issues from drugs and alcohol use. They also may have received care from other mental health services, but the treatment didn’t suit their needs. This is when we can help.
Our approach is different. Service users and their carers will work with us to help us understand them as an individual. We think creatively about what support we offer. We make sure the care and support is flexible to adapt to each persons needs. We will support recovery and independence so our service users can live a fulfilling life and avoid admissions to hospital or reduce the length of a stay, if its required. Your care will be provided by a range of professionals on our team including:
- doctors,
- psychologists,
- social workers,
- occupational therapists,
- nurses,
- support workers,
- healthy living advisors,
- harm reduction workers,
- accommodation gateway workers, and
- administrative colleagues.
We work together with our partners and other services, such as Forward Leeds, Community Links, and Touchstone, to provide the best possible care.
