Improvement Plan
To improve on the scores where we received our lowest scores, our community mental health services will:-
- Provide Service Users with support to find financial advice and find support with the cost of living
- Signpost people to the Community Hubs being introduced in 2024 as part of the Community Mental Health Transformation project, for support with finding paid/voluntary work and support to help stay in employment
- Encourage service users to give their feedback via the Trustwide Have Your Say feedback measure – or offer other ways of providing feedback
- Ensure family members and carers are involved and supported when their cared for person is in a crisis
These survey results will help us to continue to review our Community Services Strategic plans.
